We're in a blizzard right now (12-24 inches of snow expected)... I can't go anywhere. Sitting down and replying this thread might be a better than getting killed on the blizzarding road.
In USA, in medical fields, most of the advisors will pay for their graduate students. This money comes from grant money. To get this funding, they have to submit a proposal. When they get the money and a graduate student (aka a slave or, to be more polite, a cheap labor), the student will work towards the way as said in the proposal.
Along that line, the student may have found something interesting and may lead to his/her dissertation.
This is my own experience.... I have no clue at the beginning what's going to be my thesis.... Once I worked, then I knew.
