ความคิดเห็นที่ 9 First of all, I'd have to say that the author of the article really did a great job explaining the situation without letting his own opinion spilled in too much. I can sense a little of bit of negative implication, but not that much..... As a doctor, I couldn't care less about the prosperity of the drug companies. If there's any policy that would lessen the cost of the drugs, thus giving my patients better access to medication with more efficacy and less side effects, I am more than willing to support it. But then again maybe this is because I am not surrounded by drug retailers every single day and be influenced by the free gimmicks and conference trips as many other doctors are!!!! The drugs they've mentioned are truly effective, especially clopidogrel which should be readily available to every ischemic stroke or cardiovascular diseases patients who need it (but only few could afford), but we must keep in mind that HIV is a real "versatile actor" and the increasing availabilty of HIV drugs would eventually lead to increasing drug resistance, especially in Thai patients who aren't always well-educated and usually lack compliance to take their medicine with proper discipline. ความคิดเห็นที่ 10 There are two sides of a coin here... One can say that there should not be IP for drug so that the world can obtain a cheaper & better drug to - which is, actually, right. Another may say that without IP protection there will be no driven force (i.e. money) for the investor to support advance research in medical area (because, otherwise, they could not make much money from it afterward). Anyway, I think there should be both... some area that medical for preventing serious out-break that should allowed capable/qualified medical companies to freely produce without any need for license (case-by-case). ความคิดเห็นที่ 13 Today, my baby boy has a fever, so I have to stay home to babysit him... So I think this is a fine chance to express my opinions about the issue. But, first of all, I have conflict of interests directly to this issue. Thus, you can NOT consider my opinions for grant. Those conflicts of interest are; 1) I knew the person whose gave this executive decision personally and professionally and 2) my institution and myself might directly and/or indirectly receive a positive consequence upon this policy. Anyway these are my opinions and supporting evidences about the issue. To be easier for all readers, I would like to categorize the issue into 4 smaller pieces; 1. AIDS and situations in Thailand Since I have been in the US for quite some time, so given information may be out of dat. If anyone has a more recent stat about it, please share with us. In one point in time, in that period, Thailand had 60 millions population, 1 million or so were HIV positive. The disease could have become deteriorating quickly. The death from the disease comes fast and gruesome. Among those HIV infected patients are financially poor people. To effectively treat the disease, medication consisting of several kinds of drugs are required. These drugs are very expensive compared to their family income. Thus, in many families, they stopped the modern treatment into other treatments which had no scientific proof. As a result, most of them died. This is a fact that I, as a human, have to keep in mind as #1--- a lot of people are actually dying from the disease. 2. Intellectual Property and its associated rights Professionally, I'm a scientist in pharmaceutical fields. At random, 1 out of 4,000 chemicals in drug discovery process can become a drug used in clinical setting. This is time- and resource-consuming process. I saw many fellow scientists working day in and day out to overcome many fatal disease.... By this, it has never come cheap... It's a billion-dollar affair to come up with one drug. I have to pay personal tribute to this relentless effort and I believe that the company whose invested and took a risk of failing has a right to make financial benefit from their hard works. By that, I agree with patent laws and its benefits.... If my memories serve me right; in Thailand, once a patent is granted, the company can be the only body whose get benefit from the patented product for a period of 15 years. This is the second fact.... Hard workers need reward to continue their good works. 3) Efficacy & Economics of this drug As pointed out by k. ruiN (our in-house medical doctor), this drug is very effective against HIV that are resistant to other anti-HIV drugs. As pointed out by k. AmoreVincitOmnia (our in-house economist), Thailand may be able to afford the price of the drug at this current pricing. However, it might be financially practical for us to have a cheaper pricing. Two more facts from this--- There are lots of HIV people needing this drug. But it might not be financially possible for all of them. 4) Policy making Policy making is ART not science. However, science can be a tool to make decision making more reasonable and explainable. From my analyses, there are several facts being listed above; people are dying, patent & its associated rights, the drug is essential to many ailing patients and affordibility is a problem. I agree with k. AmoreVincitOmnia about "negotiations". Negotiations with the company between the Ministry of Public Health of Thailand and the highest executive level of the pharmaceutical company should be the first step to resolve the problems with a win-win-situation outcome. Meeting with an executive personel in country or regional levels may lead to misunderstanding when these people convey "the message" to the higher level. At the company standpoints, the worst-case scenario for it was a "copycat" scenario where other countries endorse a similar compulsory registration just like Thailand was going to do.
