What I'm going to tell you below is mainly from the book 'How to say it' by Rosalie Maggio na ka. I have had this book for already 5 years and it's still very useful.
Regarding to the name of a woman, if there is no clue to her marital status, use her full name without a social title, e.g. "Dear Angilina Jolie" :-) Or use "Ms." and her last name (e.g. Ms.Jolie)
In my opinion about your question "ถ้าเราไม่รู้ว่าเขาเป็นผู้หญิงหรือผู้ชาย รู้แต่ชื่อและนามสกุล เราควรเขียนคำขึ้นต้นจดหมายอย่างไรดีคะ" I think it might be safe to use full name without social title (like the one above.) I'm a woman but my Thai name is very guy-ish (manly), so I often received the letter with 'Dear Mr.." From my point of view, I found it funny because it's understandable that it's difficult for American to guess if I'm a guy/girl. But I think to play it safe, I prefer just using full name without Mr. or Ms.
In regards to complimentary close follows the body of your letter, there are lots of them you can use.
The most everyday, acceptable, and all purpose complimentary closes are: Sincerely, yours truly, Sincerely yours, Very sincerely yours, Very sincerely. You cannot go wrong with one of these.
For a highly formal letter involving White House, diplomatic, judical, or ecclesiastical correspondence, use: Respectfully yours or Respectfully. An informal letter in the same instances use: Very respectfully yours, Yours respectfully, or Sincerely yours. In formal letters to members of Congress, senators, high-ranking politicians and government figures, priests, rabbies and ministers, use: Yours very truly. The informal form is: Sincerely yours.
For most formal letters-regular business and personal-choose from among: Sincerely, Sincerely yours, Yours sincerely, Very sincerely yours, Very sincerely, Truly yours, Yours truly, Very truly yours, Yours very truly, Very cordially yours.
Informal closes include: Sincerely , Sincerely yours, Cordially, Cordially yours, Yours codially, Faithfuly, Faithfully yours, Yours faithfully, As ever, As always, Yours, Best regards, Kindest regards, Warmest regards, Cheers, Your friend, Be well, Until next time, Love,etc.
Complimentary closes somewhere between formal and informal include: With all kind regards, Warm regards, Best regards, Best, Best wishes, With best wishes, Will all best wishes, Cordially, Sincerely, All the very best, With every good wish, Warm personal regards.
For me, the most often complimentary closes I use are: Best Regards/Kind Regards/Cheers (to my colleague), Sincerely yours/Best Regards (to my customer)
