Some of the terminology here is getting a little confused.
Again, I am currently in the PhD program in Bioengineering at the University of Maryland. All students here are provided financial support as soon as they enter the program. The Bioengineering Department provides financial support for the first year. After that, the student selects an advisor, and the financial support comes from the research funding. All reputable science and engineering doctoral programs in the US pay for their students to attend. They will pay your tuition, and give you a living stipend (ranges from $20,000 to $30,000 per year). All students accepted into the PhD program are covered, regardless of their current level in the program. (This is why you want to apply to the PhD program, not the Master's program) Don't trust any program that wants you to pay to study there for engineering PhD in the US. It is probably not a respected institution.
While we have a few students who already had a Master's before coming, the majority entered the program with only a Bachelor's degree. We have people with a Bachelor's in bio or biomedical engineering, but also biology, chemical engineering, mechanical engineering, electrical engineering, and probably some others. The important thing when you apply is that you have strong reasons for wanting to study the subject.
The schedule for a student is as follows. After the first semester, they take the qualifying exam (called the Research Aptitude Test). This used to be a five hour written test, but now most schools here are making this into a research proposal. If you don't pass the RAE, you have one more chance to take it the following summer.
After the first year, you will be working with your research advisor. You then have a choice: You can do a Master's thesis, followed by a PhD, OR, you can go directly and do your PhD. Most students choose to go direct to the PhD. The next step in the process is the dissertation proposal, in which you describe what your dissertation topic will be. After successfully passing the proposal, you are now a PhD candidate. The final step of course is the dissertation.
