You need to practise opening your mouth to speak at all occasions that you have a chance to speak. Even if you don't have a chance to speak often, you need to practise speaking by yourself. The best way is to d/l podcast from various website and then repeat what you have heard, using "pause" function of windows media player to do so.
Learning does not help if you don't speak or has a chance to speak.
Let me tell you a story.
A Thai woman graduated with a BA from Australia. Of course she was fluent in both reading/writing/speaking. Three years later she confessed to me when we met for a re-union that she could no longer speak English fluently even though she could understand everything she heard. She worked in Thai bureaucracy and did not have a chance to speak English at all. She got the same problem with you. Her thought process was all Thai.
