ช่วยแปลกผลเอ็มอาร์ไอให้หน่อยคะ ตรงใหมที่บอกเราว่าอาจจะเป็นมะเร็ง

มีอาการปวดท้องน้อยกับปัสสาวะเป็นเลือด ตรวจหานิ่วก็ไม่เจอ ตรวจมดลูกด้วยอัลตร้าซาวด์และส่องกล้อง ตรวจมะเร็งปากมดลูกไม่เป็น แล้วหมอก็สั่งทำ CT สะแกนพอผลออกมา หมอก็สั่งทำ เอ็มอาร์ไอยังไม่ได้เจอหมอเลย แต่คุยกันทางโทรศัพท์ พอผลออกมาก็เจอหมอ หมอสงสัยว่าจะเป็นมะเร็ง ส่งตัวฝนไปเจอหมอสูติ พออ่านแล้วไม่เห็มว่าจะมีตรงใหนที่บอกว่าจะเป็นมะเร็ง หรือว่าจะเป็นคำศัพท์เฉพาะ
ฝนเคยผ่าตัดหน้าท้องเพือเอาลูกออก 2 ครั้ง ครั้งสุดท้ายก็ ตุลาคม 2552
แล้วมีอาการปวดท้องน้อย ประมาณเดือน มกราคน ต้นเดือนกุมพาไปหาหมอ ถึง ตอนนี้
อาการปวดเริ่มมากขึ้น ตอนนี้ฝนกลุ้มใจมากคะ ยังไม่ได้เจอหมอคนใหม่ ก็จะนัดได้อาทิตย์หน้า

Findings :The  ureus has retroflexed morphology near the fundus.
The anterior wall of the myomertrium is thin. There is a tubular strucre in the typical location of a urachal remnant that extends from the anterior  abdominal wall inferiorly along the anterior aspect of the myometrium. This is best  visualized on post –contrast coronal series9,image27 and axial series7,images9-17. The appearance along the cephalad aspect has a more tubular appearance . There is brisk enhancement. This process dose become contiguous with anterior wall of the uterus. Its communication with  the urinary bladder is not clearly identified . The patient iss status podt cesarean section and there is thinning of the anterior uterine myometrium . An IUD is in place. The ovaries appear normal  with bilateral,small,normal follicles. No pelvic adenopathy is identified . Thewe are postsurgical changes in the lower anterior abdominanl wall consistent with of cesarean section . Threr is no significant free fluid.

Impresion :
Enhancing tubular structure extending from the anterior abdominal wall inferiorly ,along the anterior aspect of the myometrium,become a triangular- shaped structure adhrernt to the anterior wall of the myometrium as it travels inferiorlv. Its communicationwith the urinary bladder is not well visualized on this examination.A  urachal remnant  possibly including potential urachal carcinoma,cannot be excluded . The adherence of this structure to the myometrium may be a sequel of a cesarean section .The concern for urachal abnormality is increased , given the patient’s clinical presentation of hematuria from the more recent CT.
