The term "Palestinian" first came into use during the Roman period:
"Palestina" was the name Rome gave to its (predominantly Jewish) colonies in the Levant. The name was later adopted by the British during the mandate period, 1917-1948. For a time, it referred to all those living in this region, including Jews. During the first period of Israel's existence, 1948-1967, "Palestinian" was not a term much used in reference to the people now known as the Palestinians.
Gazans were considered Egyptians; West Bank Arabs were considered Jordanians. At most, they might have been called "Palestinian Arabs" a reference to location, not identity. The term gained currency following the 1967 Six Day War, when Arab leaders, particularly Egypt's Nasser, decided to adopt a guerrilla approach in his efforts to destroy Israel. He anointed Yasser Arafat head of this guerrilla effort, who adopted the rhetoric of national liberation struggles of the sort popular throughout the Third World then.
Thus the "Palestinian people" were born.
As late as 1974, it was possible for Israeli Prime Minister Golda Meir to claim that the very term "Palestinian" was a fabrication. But that became increasingly difficult as Arafat and the PLO won international acceptance, particularly at the UN. Since then, it is fair to say that a genuine "Palestinian" identity has taken shape, the result of living for decades under Israeli rule or in harsh refugee camps throughout the Arab world. (*Harsh because the Arab states, with breathtaking cruelty and cynicism, never allowed these refugees to assimilate into their populations.*) That identity has been strengthened by the first intifada of 1987-1992, and by the current wave of violence.
To a large extent, Palestinian identity is a creation of Arafat, and it will be interesting to see if it can survive him. Perhaps it can, and I don't see much point in denying Palestinians their unique identity. At the same time, it's important to keep in mind how recently their identity was formed...
